The Titan submersible, which was released by the Coast Guard Marine Board of Investigation, was found on the bottom of the Atlantic Ocean after an implosion that resulted in the deaths of all five crew members.
The video footage of the vessel's tail cone resting on the ocean floor was the conclusive evidence needed to determine the catastrophic loss following the incident on June 18, 2023.
The Titanic's wreckage was hit by the Titan less than two hours after its descent.
If OSHA had investigated a complaint filed by OceanGate's former operations director, David Lochridge, the Titan tragedy could have been prevented.
He expressed deep disappointment as a seafarer regarding the system designed to safeguard both seafarers and the public.
Lochridge refused to greenlight manned tests of the submersible over safety concerns.
Lochridge testified that eight months after filing an OSHA complaint, a caseworker informed him that the agency had not started investigating it yet, and there were 11 cases in front of him. At that point, OceanGate was suing Lochridge, and he had filed a countersuit.
After 10 months of filing the complaint, he decided to withdraw from the case. The Associated Press reported that the case was closed and both lawsuits were dismissed.
The Titanic had been on several dives to the Titanic going back to 2021, the AP reported.
The Titan submersible hearings by the U.S. Coast Guard started on Monday and will continue on Thursday in Charleston, South Carolina.
